I certify that, to the best of my knowledge:
  • I am enrolled (or accepted for enrollment) at The University of Texas at San Antonio; 
  • I am not enrolled in either an elementary or secondary school; 
  • I am maintaining satisfactory progress in my course of study; 
  • I do not owe a refund on grants previously received at any college/university and I am not in default on any student loans from a college/university or the U.S. Department of Education; 
  • I have filed or am eligible to file a Free Application for Federal Student Aid (FAFSA); 
  • I am a citizen or national of the United States, a permanent resident of the United States, or able to provide evidence from the Immigration and Naturalization Service that I am in the United States for other than a temporary purpose with the intention of becoming a citizen or permanent resident; 
  • I have a valid Social Security number (unless I am from the Republic of the Marshall Islands, Federated States of Micronesia, or the Republic of Palau); 
  • if required, I have registered with Selective Service; 
  • I have a high school diploma or GED, or I completed high school in an approved homeschool setting; 
  • I have not been convicted under federal or state law for possession/sale of illegal drugs for conduct that occurred while I was receiving federal student aid to attend a college/university or, if I have, I have regained eligibility by completing a drug rehabilitation program; 
  • and if I have been convicted of, or pled nolo contendere or guilty to, a crime involving fraud in obtaining federal student aid, I have repaid those funds.

Statement of Need

Please write a brief, typed statement explaining your emergency and describing how the funds will be used. Itemize each cost and provide a total of the requested amount. Supporting Documentation: Please attach evidentiary documentation to support your reason for requesting an emergency grant. Examples include medical bills, bank statements, letter from your employer stating your job loss, letter from your landlord/bank about rent owed or threat of eviction, police report documenting theft, or letter from your childcare facility.
